Before we can legally send it out to you, we need to receive a completed Statement Of Medical Necessity (or prescription equivalent) from a medical professional with a National Provider Identifier (NPI). Directions are as follows:
- Download And Print Out Your Statement Of Medical Necessity By Clicking On This Link
- Have your medical provider complete the form. Alternatively, they can use this form as a guide for writing the prescription on their own letterhead. Note: For the latter option, please stipulate that the prescription states “Prescribing FIRSTtx for pain relief” and shows your doctor’s NPI.
- Mail, email or fax it back to us (all contact information can be found below as well as on the Statement Of Medical Necessity).
- As soon as we receive your Statement Of Medical Necessity, we’ll finish processing your order and we will send your FIRSTtx to you right away.
